New Mobile, Email Surveys Start This Fall

To help us continue to improve your patient experience, Bastyr Center for Natural Health has implemented a new survey system you can expect to see in your email inbox or on your smart phone.

In November, we will be sending a new annual survey to patients who provided an email address on their intake forms. This survey is expected to take about 15 minutes and will include questions about your patient experience and care.

Then in December, we will restart our monthly survey to patients who visited the clinic in the month prior and who provided their mobile phone number on their intake forms. This survey will be a short, five-question survey asking about their experience.

While your participation is voluntary, your responses on these surveys will help us enhance our services to better address your needs and improve your patient experience.

The confidentiality of every participant in this survey is of the utmost importance. Your responses are confidential and will be analyzed collectively with other participant responses.
